About Your Medical Bill
mEDICAL INSURANCE BILLING
Medical Insurance claims can be confusing. Insurance companies may delay payment to us for months, and we do not bill you for your balance until your insurance makes their paymet. Our costs for medical billing typically is over 10 percent.
As always you can contact our billing department at 888-312-3007. Below are some common billng terms.
Copayment
This is a set dollar amount you pay for the covered service.
Coinsurance
After your Annual Deductible is met, this is the shared cost for covered services that you pay with your insurance company. This is usually a percentage of the payment set by your health insurance for that specific service. For example, if you have an outpatient service that your insurance says is paid at $100, if your coinsurance is at 20%, then you would pay $20 and the insurance covers $80.
DEDUCTIBLE
A Deductible is the amount of money you pay for covered services before your health insurance starts to pay. This typically resets at the beginning of the year.

INSTRUCTIONS
- We do not accept email, fax or telephone requests for medical records.
- Requests must be in writing and mailed to: Medical Records, Suite A 180 E Leland Rd, Pittsburg, CA 94565
- It must include three items:
- a written sign authorization from the patient within 12 months for release of Medical and Psychiatric Records
- a check for $100 made out to "Vital Total Health Medical Group Inc"
- a destination and method where you want the records sent to.
